Membership term must be for 3 months. First's month Membership fee is due at the time of membership. Each additional month your credit card on file will be charged on the 5th or 15th day of the month unless stated otherwise. Need to skip a month? No problem, notify us between the 1st and the 4th. Memberships can not be skipped more than three times in a row or this will result in full cancellation and a $25 re enrollment fee will be charged. Memberships can be cancelled at anytime with a 30 day written notice but will be charged an early cancellation fee equal to one month's membership fee if cancelled before three consecutive months. Cancellation Policy still applies to all memberships. Any no show or appointment canceled with 24 hours is subjected to additional fee.

First's month Membership fee is due at the time of membership.

Each additional month your credit card on file will be charged on your selected day between the 25th -30th of the month unless stated otherwise.

Need to skip a month? No problem, notify us between before the 15th for either Membership (Sqin Maintenance can not be skipped before three months from start date) can not be skipped more than three times in a row or this will result in full cancellation and a $25 re enrollment fee will be charged.
